AA Form – Three Entry Fields and One Error!

I recently completed a short form (Requesting only my name and my email) on AA (New Zealand Automobile Association) website to enter into a draw. Have a look at the following screen shots and think how you could design a better form!

And then, after filling the form, I was looking for the submit button…

At first sight, the SUBMIT ENTRY is not obvious when you have the whole page in front of you.

Oops, I got an error; “Please enter email”. Well, I did. Why somebody decided to not display labels beside the three fields if the website is not able to manage several ways of entering data?
